Fatal fail — script violation

A fatal fail of type script violation triggers four downstream actions in QUALICORE:

The evaluation is locked at 0%.
An escalation is auto-raised on the supervisor's queue.
The supervisor is required to schedule a coaching session within 24 hours.
HR is notified if the agent has more than two fatal fails in a rolling 30-day window.

QA tip:* When you mark a fatal fail, capture the timestamp from the call recording in your QA comment. Disputes are much easier to resolve when the evidence is precise.
