Auto-escalation: script violation

QUALICORE evaluates five trigger algorithms after every QA submission:

  1. Fatal fail — any criterion of type script violation.
  2. Repeat fails — the same criterion failing across 3+ evaluations in 30 days.
  3. Score collapse — a 25% drop in rolling 14-day average.
  4. Compliance window — any criterion tagged POPIA, AML or PCI-DSS failing.
  5. Supervisor SLA — coaching plan not opened within 48 hours of escalation.

Each trigger has its own escalation queue. Supervisors can re-route or close with a documented reason.
